After Signup: Complete your Patient Intake Form
Just like a doctor’s visit, you’ll need to complete your patient intake form where you will be required to upload your ID and a photo of yourself. Some States require a short video. This information is protected in our HIPPA-compliant patient portal and in necessary in order for the doctor to complete your prescription.

NOTE! If you’re a Direct Meds customer and have paid for your medication, please make sure to log in to your patient portal and complete your medical intake form. Your medication will not be able to ship until this form is fully completed.
Your patient intake form helps us understand your health history to provide the best possible care. Just like a doctor’s visit, this form includes a few simple steps and takes only a few minutes. Once you’re logged in, you’ll be prompted to complete all required fields, including uploading a photo of your ID.
Please remember: if your ID hasn’t been uploaded, your patient intake form is not yet complete. This step is crucial to move forward with your treatment and ensure compliance with telemedicine regulations.

1 Log in to your patient portal using your email and password from sign-up.
2 Complete the health history section.
3 Upload your ID and a photo of yourself (or a short video, depending on your state’s requirements).
